CNA/Transportation Driver Requirements:

  • Active CNA certification in the state of PA

  • Ability to lift up to 50 lbs and assist with physical transfers.
  • Experience with elderly or disabled populations.

CNA/Transportation Driver Responsibilities:

  • Transport residents to medical appointments, dialysis, and other approved outings using facility vehicles.

  • Ensure wheelchair-bound residents are properly secured.
  • Maintain daily vehicle inspection logs and notify maintenance of issues.
  • Assist nursing staff with resident care duties when not driving (ADLs, ambulation, toileting, etc.).

  • Comply with HIPAA and infection control standards at all times.

POSITION SUMMARY:

The Physical Therapist organizes and conducts physical therapy programs for the facility to facilitate rehabilitation. 4          Consults with other of the rehabilitation team to select the most appropriate therapeutic regimen consistent with the needs and capabilities of each resident;

5          Instructs resident in the skills and techniques required for participation in mobility activities, and evaluates client progress, attitudes, and behavior as related to rehabilitative potential;

6          Adapts program according to the needs of individual resident;

7          Promotes a safe environment and enforces the elimination of fire and safety hazards;

8          Attends and contributes to resident care, staffing conferences and other related meetings;

9          Supervises ive personnel in accordance with state and professional standards;

10       Conducts training programs and participates in training medical, nursing and other personnel in physical therapy techniques and objectives;

11       Orders supplies and equipment as necessary;

12       Cleans, maintains, identifies, and reports the need for repair of equipment as necessary;

13       Ensures that residents and families receive the highest quality of service in a caring and comionate atmosphere that recognizes the individuals’ needs and rights.

SPECIFIC EDUCATIONAL/VOCATIONAL REQUIREMENTS:

Graduate of a physical therapy curriculum accredited tly by the Council on Medical Education of the American Medical Association and the American Physical Therapy Association and certified by the American Physical Therapy Association under its requirements.

JOB SKILLS:

1          Good organizational skills and time management skills.

2          Good verbal and written communication skills.

3          Licensed and/or eligible for licensure as required per the State.

PERFORMS RELATED DUTIES:

1          Interacts with residents, families, visitors and employees.

2          Carries out other tasks as requested in situations where hands-on intervention/participation may be required.
